Dear Dr. DeLuise,

I hope that all is well with you. I wanted to reach out to you concerning my grade on the most recent assignment. I feel that I invested a great deal of time meeting the assignment requirements, doing research, and writing the assignment, but my grade was far less than what I had hoped. I do not remember seeing a link to the rubric for this assignment, and I felt that I had met, and in some cases, exceeded the requirements, but I do not feel that my grade adequately reflects my effort.

I would be very grateful for any constructive feedback that you could offer to help me improve my academic standing in this class. I am very motivated to excel in my classes and not afraid to go the extra mile in my assignments. While I am sure I invested a great deal of time and energy into the last assignment, my grade makes it very apparent that my direction may have been askew from the requirements of the course.

Dear Dr. DeLuise,

I hope that you are doing well. I was recently reviewing my midterm grades online, and I noticed that my grades are much lower than I anticipated. In reviewing the grade book, I noticed that a substantial amount of points had been deducted from every assignment. I took a moment to review the course guide located in the student center to try and gain some understanding before reaching out to you. After all of my research, I am still uncertain why I am performing so poorly.

I am reaching out to you to schedule an appointment during your regular office hours to discuss my shortcomings and to establish a plan that would help me to pass this class with a more respectable grade. To prepare for our meeting, I will have all of the assignments I have completed and submitted printed out along with any information provided about the assignment. My sincerest hope is that you might be able to help me find the gaps and form a plan to fill the gaps for more productive academic success.

I am very grateful for any guidance that you can offer that will lead to more exceptional accomplishments in the second half of the class.
