Language/ Communication By May 29, Huang will ask question of others regarding topics initiated by self or others to sustain conversational turn-taking 4/5 opportunities during class or after class. By April 17, Huang will spontaneously request assistance, help or additional information given visual prompts 4/5 opportunities during class. During or after class, Huang will ask questions to other students in in different environments. Teacher aide will observe the student at all times and will record observations in data sheet. During class, Huang will ask for help or assistant by using visual cues. Teacher or teacher aide will record the number of times he is able to achieve this goal. Teacher will be observing the student using data sheet.
Social/ Behavioral Skills By April 30, Huang will be able to initiate and maintain peer interaction with others during content area, 4/5 opportunities. By May 15, Huang will engage in conversational turn-taking with others across 3-4 conversational turns throughout the school day, 4/5 opportunities to do so. During classroom activities, Huang will work in teams of 2 to 4 students. He will engage and communicate verbally during these activities. Teacher will be observing Huang’s interaction and will use a data chart to record data. During or after class, Huang will engage communication with other students while taking turns. Teacher aide will record in a data sheet the times Huang initiates and maintains a conversation while taking turns.
Classroom Management Skills By April 17, Huang will raise his hand and wait to be called 4/5 opportunities during classroom activities. By May 1, Huang will engage in appropriate turn-taking skills with help of another peer and wait for his turn 4/5 opportunities in classroom setting. During classroom activities, Huang will participate in activities and will raise hand if he needs help or to respond a question asked by the teacher. The student will wait calmly without disrupting others until he is called. As measured by teacher observation.During classroom activities, Huang will engage in turn-taking either by using class material or to responding questions. Student will be assessed by progress monitoring and teacher observation.
Mathematics By May 1, Huang will be able to identify numeric expressions in a word problem 4/5 times during classroom activities. By April 24, Huang will be able to collect data from story problems and create charts 4/5 opportunities during classroom activities. Huang will be assessed and monitored by student work samples and classroom progress.
Literacy By April 24, Huang will be able to identify what happened first, in the middle, and last from previous story, past event, and situation by work assignments or verbally. He will achieve this goal during content area 4/5 opportunities. By May 1, Huang will be able to narrate information in chronological order by verbal response or activities from 4/5 opportunities. During classroom activities, Huang will be assessed by teacher using student work samples and teacher observations. Huang will be assessed by teacher’s observation and data sheet.
Self-help Skills By May 30, Huang will be able to participate in task/ activities to complete by exhibiting appropriate behavior 85% of the time, during classroom activities.By April 17, Huang will independently ask for a break giving visual and verbal prompt 80% of the time during classroom or outside of class. Huang will be assessed and monitored by using progress monitoring and teacher observations. Data will be collected every week to record student’s progress.
