Overview: “Information Retrieval Paper – Part 1”

This Module you will begin to develop an Information Retrieval Paper. This Assignment focuses on identifying appropriate evidence-based nursing practices related to specific problems you may identify as you work in the healthcare/workplace setting.

The goals of an Information Retrieval Paper are to (1) practice using APA format, (2) summarize and examine the strengths and limitations of research articles, and (3) prepare you for the Nursing Research Course where you will write a research paper using the skills you have learned completing this Information Retrieval Paper.

As a part of your Information Retrieval Paper, you will begin development of a research question using PICO format. Chapter 9 of the Role Development text book explains PICO. Be sure to develop your research question and then just below your research question delineate what the P, I, C, and O components of your question are.

For this Module 3 Assignment you will prepare a Title Page in proper APA format for your Information Retrieval Paper, identify three peer-reviewed articles, and summarize each article using APA format.

It may be helpful to look at the outline for the entire paper before you begin this Module’s Assignment. Assignments in this Module plus in Module 4 and 5 will address the Information Retrieval Paper.

Part 1: Research Problem and Question using PICO Format

In the space below, identify the clinical problem in the workplace setting that will serve as the focus of your information retrieval paper. Make sure that the problem is one that would be under your complete control as a nurse.

Clinical Problem (Type in box below)

Regarding patient care on the medical-surgical floor at my facility, how will the implementation of hourly rounding, compared to current sole use of the John Hopkins Fall Risk Assessment Tool help to reduce patient falls.

In the space below, compose a preliminary research question about the clinical problem that you identified in the workplace. Use this criteria in composing a preliminary research question:

– Question must relate to a clinical problem in the workplace.

– Question cannot be answered with a simple yes or no.

– The issue to be studied is under your complete control as a nurse.

– The question is open ended.

Delineate the PICO components of your question.

State your question

Then under the question

P (problem) =

I (intervention) =

C (comparator) =

O (outcome) =

Preliminary Research Question and Identified PICO Criteria (Type in box below)

P: Patient falls on medical-surgical floor

I: Implementation of hourly rounding sheets in patient rooms, use of bed alarms

C: Use of John Hopkins Fall Risk Assessment Tool to identify and place fall risk band on patient, but no hourly rounding.

O: A decrease in the number of patients falls on medical-surgical floor

Compose a rationale that explains why you chose the research question. (Type in box below)

Rationale for Selecting the Research Question (Type in box below)

I selected this research question because, I currently work on the medical-surgical floor and lately there has been an increase in the number of patient falls. Recently we had a total of 4 falls in one week and had an incident where a patient’s family member was turning off the bed alarm to encourage the patient to ambulate to bathroom instead of using a bed pan. The current process used at my facility is using the John Hopkins Fall Risk Assessment Tool to identify patients at admission. If the patient is a high fall risk, a fall risk band is placed on the patient and the bed alarm is activated. By the time the bed alarm is activated, it usually results with the patient already on the floor even with the call light accessible.

After completing this portion of the Assignment, go to this module’s Discussion,
and follow the directions for sharing your research question. After receiving feedback

from your colleagues, revise your research question, if necessary, in the space provided.

Revised Research Question (Type in box below)

Complete after participating in the Module 3 Discussion. Type your revised Research Question in the box provided below.

Regarding patient care on the medical-surgical floor at my facility, how will the implementation of hourly rounding, compared to current sole use of the John Hopkins Fall Risk Assessment Tool help to reduce patient falls.

P: Patient falls on medical-surgical floor

I: Implementation of hourly rounding sheets in patient rooms, activation of bed alarms

C: Use of John Hopkins Fall Risk Assessment Tool to identify and place fall risk arm band, but no hourly rounding

O: A decrease in the number of patients falls on the medical-surgical floor

Part 3: APA References and Summaries

Select three peer-reviewed, evidence-based articles that address your research question.

Compose an overview about your choice of the three articles. Answer these questions:

Where did you search for articles?

How did you decide what articles to include?

Post references to the three articles in APA format.

Summarize the article and how it addresses your research question.

Overview

Where did you search for articles? How did you decide what articles to include?

I choose to search for peer-reviewed, scholarly articles within the 5-year timeline through use of the UTA online libraries and the Google Scholar search engine. I decided on my selection of articles that were current in time and those that could provide me with evidence-based practices of knowledge regarding my research topic.

Article 1

In the space below, post the reference for the first article in correct APA format.

Goldsack, J., Bergey, M., Mascioli, S. & Cunningham, J. (2015). Hourly rounding and patient falls. Nursing, 45(2), 25-30. doi: 10.1097/01.NURSE.0000459798.79840.95

In the space below, write a summary paragraph in APA format. Use scholarly writing to describe the article, the decision-making process you used to select the article, and how it addresses your research question. Use professional writing style.

This article addresses the issue of patient falls and acknowledges the fact that patient falls are an increasing problem within healthcare. A study was conducted over 30 days to evaluate the impact of hourly patient rounding on two different units. Statistical data collected from the study demonstrated a significant decrease in patient falls and use of call bells with hourly rounding of patients. The author further implies patient hourly rounding is more beneficial when staff and leadership personnel are involved as a team and concludes without this team environment hourly rounding has no effect on fall reduction. This article was selected as a source for my research question because an actual study was implemented focusing on reducing patient falls to provide evidence of reduction.

Article 2

In the space below, post the reference for the second article in correct APA format.

Morgan, L., Flynn, L., Robertson, E., New, S., Forde-Johnston, C., & McCulloch, P. (2017). Intentional Rounding: a staff-led quality improvement intervention in the prevention of patient falls. Journal of Clinical Nursing. 26(1-2), 115-124. doi: 10.1111/jocn.130401

In the space below, write a summary paragraph in APA format. Use scholarly writing to describe the article, the decision-making process you used to select the article, and how it addresses your research question. Use professional writing style.

This article explains that patient falls are a common occurrence regarding treatment in a hospital. Patient falls result in increased hospital cost, patient length of stays, morbidity, and mortality rates. The author reports an estimate of approximately 282,000 falls reported each year with 19,579 cases resulting in injury Morgan, et al., 2016) The author describes a study on hourly rounding where caregivers are required to sign a log sheet posted on the patient’ door. Most falls are the result of a patient in pain, positioning needs, or elimination needs such as access to the toilet. With hourly patient rounding there is an increased presence of personnel to help when these patients are in need. The author provides evidence of the study regarding hourly patient rounding resulting in a 50% reduction in patient falls.

I selected this article because a study was performed resulting in a reduction of patient falls by implementing hourly rounding, which addresses my research question. This article was also selected because the author explains the most common rationales of why patients fall. Another determining factor for selecting this article is the effect falls cause to patient health and costs to facilities.

Article 3

In the space below, post the reference for the third article in correct APA format.

Chu, R. (2017). Preventing in-patient falls: the nurse’s pivotal role. Nursing, 47(3), 24. doi: 10.1097/01.nurse.0000512872.83762.69

In the space below, write a summary paragraph in APA format. Use scholarly writing to describe the article, the decision-making process you used to select the article, and how it addresses your research question. Use professional writing style.

This article reports patient falls as being among the top 10 sentinel events investigated by The Joint Commission. The author describes consequences of patient falls regarding the patient and the hospital. The author goes further into detail to describe a 3-year study reporting age groups most associated with falls and lists contributing factors such as diseases leading to falls. Medications, aging, and impairments are also described as being risk factors associated to patient falls. The author further explains that some reasons falls exist is because patients do not use call lights, staffing shortages, and environmental factors such as low lighting or tripping hazards.

The author suggests preventative measures such as activation of bed alarms, hourly rounding, patient and family education, and use of fall risk armbands. Nurse’s are the frontline for providing education, identifying risk factors, and implementing appropriate evidence-based interventions. Nurse’s are key players in promoting patient outcomes and high-quality patient care. I choose to article because the author explains patient health conditions and contributing factors relevant to patient falls that can easily be remediated in prevention.
